PROOF OF CITIZENSHIP
Proof of U.S. Citizenship. You will need to provide one of the following.

Original Government Issued Birth Certificate. Hospital and religious certificates are not accepted.
Previous U.S. Passport issued for full term (5 to 10 years) that has not been damaged in any way.
Original Naturalization Certificate
Important Notes:
Photocopies and notarized photocopies are NOT accepted, all certificates must be originals

   
 
     
    PROOF OF IDENTIFICATION
Proof of Identification. You will need to provide:

A clear copy of your U.S. Driver's License. You will have to have the original with you when you sign the application form.
Important Notes:
State Ids and permits are NOT accepted.

    
         
    PASSPORT PHOTOS
Two identical Passport Photos taken within the past 6 months.

These should be 2x2 white background photos that meet
Passport Regulations.

Eckerd's and Mailboxes Etc are examples of stores that take passport photos. If you don't have one of these stores located near you, try looking in your local Yellow Pages under "Photographers - Passport"
